1. By completing a PDF form in the link above (please read the instructions below carefully):
Instructions for completing the form
You will need at least Adobe Acrobat Reader version 7, but suggest you download the latest version here: Adobe Acrobat Reader
– First, you must save the PDF form (above) to your computer. Do this by right clicking the thumbnail, and selecting ‘SaveLinkedFileAs’… or ‘SaveTargetAs’… or ‘DownloadLinkedFile’…
– Open the form and enter your details in the boxes – some of the boxes must be filled, and you will not be able to send the form until they are all completed.
– When you have finished, you have three options:
- Click the ’submit by e-mail’ button at the bottom of the form, and the information you have entered will be sent straight to us. This uses the email programme you have on the computer.
- If you use a web-based account (like Hotmail, Yahoo or Gmail), or if you are not doing this at your home computer, then choose the ‘Internet Mail client’ option. You will be prompted to save the completed form to your computer (you can save it in ‘my documents’). Then, attach the saved file in an email to admission@sdcol.org.uk.
- Complete the form on your computer and print it out, posting it to the office.

If you click the image, it will open in a new window or tab so you can see what it looks like. Please do not try and fill it in from inside your browser – it may not work properly. Save it to your computer first and then complete it from there.
2. Alternatively, you can print the blank PDF form, complete it by hand and post it to the office – a member of the team will pick this up and enter your details electronically. Please ensure your email address is written clearly – if it is entered incorrectly, you will not receive the automated email containing a full application form and reference forms which must be completed in order to make a successful application.
3. You can call the office on 0121 458 5240, where a member of our team will enter your details for you.
Once you have registered your interest in the course of your choice, you will receive an email containing a full application PDF form and PDF reference forms. You will also receive an application pack in the post.
